Established patients can schedule an appointment by calling 307-315-6133 between the hours of 8 AM to 5 PM, Monday through Thursday and 8 AM to Noon on Friday.

If you have been to another doctor's office within the past 3 years we will need to obtain your old records from that office. Please fill out the Records Release form and fax or email it to our office preferably prior to your first visit so that the doctor may review your history prior to seeing you. You may also bring in the form filled out at your visit or arrive early and fill it out prior to your appointment.
We also ask that you fill out a New Patient Intake Form prior to your first visit and this may be faxed or emailed to the office as well. This is a very comprehensive questionnaire about your medical, psychological, and social history. Filling out this form may seem time consuming and cumbersome but it will help your doctor determine your risks for certain illnesses and conditions and possibly catch them early or aid in prevention.
What is the Patient Portal?
Patient Portal allows you access to your medical record anytime and anywhere including your medications, allergies, past medical history, surgical history, and any lab tests or radiology reports performed out of our office. It will have the option of performing an e-visit with your doctor or scheduling your own appointments or requesting medication refills as well. Patient Portal is only available to ESTABLISHED patients of Rocky Mountain Family Medicine. Please inquire about this free service during your appointment.
